Applications for the MAX
Scholarship are based on the 2009 CSET application form which is available at http://cset.mnsu.edu/sacc/scholarships. Please use the MAX Scholar addendum form available at http://cset.mnsu.edu/max/MAXaddendum09.pdf to complete your application. Applicants should plan to submit the following information:

Recommendation letters requested in the CSET application must be as follows:

One recommendation form from your faculty advisor that includes information about your commitment to your major and being on-track for graduation. Your recommendation forms must include additional comments or an attached letter.

One recommendation form from a professor with whom you have taken a class that includes information about your academic ability and leadership qualities. Your recommendation forms must include additional comments or an attached letter.

One optional recommendation form or letter of recommendation from an employer that includes information about your leadership qualities.

Recommendations must be submitted in envelopes that are sealed and signed by the author.

A completed CSET application form (including a one page essay describing your educational goals)

A completed MAX Scholar addendum form

A one page essay addressing how you will contribute to the diversity of the MAX Scholar group.

A transcript (and a transfer audit/evaluation if you are currently an MSU student and transferred credits to MSU)
In addition, please submit your FAFSA form as soon as possible. A program requirement is eligibility for need-based financial aid.
