If you have a declared major and an assigned advisor in the College of Science, Engineering and Technology and are planning to register for upper level (300 or 400 numbered courses), you will need to complete the form below. Please fill it out, have your advisor sign it, and turn it in with your academic record(s)* to the Advising Center in Trafton C-125.
